Az ibrutinib ter\u00e1pia a relab\u00e1l\u00f3\/ refrakter CLL kezel\u00e9s\u00e9ben az ut\u00f3bbi \u00e9vek nagy \u00e1tt\u00f6r\u00e9s\u00e9nek tekinthet\u0151, azonban m\u00e1ra a hossz\u00fa kezel\u00e9si id\u0151 eredm\u00e9nyek\u00e9nt egyre t\u00f6bb betegn\u00e9l kell sz\u00e1molni a rezisztencia kialakul\u00e1s\u00e1val, melynek mechanizmusa m\u00e9g csak r\u00e9szleteiben ismert. <\/p>\n\n\n\n<p>A ter\u00e1pia \u00e1ltal induk\u00e1lt evol\u00faci\u00f3 jelens\u00e9g\u00e9nek behat\u00f3bb megismer\u00e9s\u00e9re k\u00f6zlem\u00e9ny\u00fcnkben az ibrutinib ter\u00e1pi\u00e1ban r\u00e9szes\u00fclt 30 magyar betegt\u0151l a ter\u00e1pia megkezd\u00e9se el\u0151tti, valamint ut\u00e1ni mint\u00e1kat hasonl\u00edtottuk \u00f6ssze 30 g\u00e9n mut\u00e1ci\u00f3s st\u00e1tusz\u00e1t vizsg\u00e1lva. A ter\u00e1pia hat\u00e1s\u00e1ra a daganatos sejtek dinamikus klon\u00e1lis v\u00e1ltoz\u00e1son mentek kereszt\u00fcl: egyes g\u00e9nek mut\u00e1ci\u00f3inak el\u0151fordul\u00e1sa cs\u00f6kkent, m\u00edg sz\u00e1mos mut\u00e1ci\u00f3\u00e9 n\u0151tt. Az ismerten ter\u00e1pia rezisztenci\u00e1t okoz\u00f3 g\u00e9nek, a <em>BTK<\/em> \u00e9s <em>PLCG2<\/em> eset\u00e9ben t\u00f6bb rezisztenci\u00e1t okoz\u00f3, ezid\u00e1ig nem publik\u00e1lt mut\u00e1ci\u00f3t is azonos\u00edtottunk. A <em>BTK<\/em> \u00e9s <em>PLCG2<\/em> mut\u00e1ci\u00f3k kiz\u00e1r\u00f3lag a kezel\u00e9s ut\u00e1ni mint\u00e1kban fordultak el\u0151, \u00e9s ezen mut\u00e1ci\u00f3k megjelen\u00e9se m\u00e1r t\u00f6bb mint 10 h\u00f3nappal a klinikai relapszus kialakul\u00e1sa el\u0151tt kimutathat\u00f3 volt.
